Service Eligibility
Any individual with a physical, mental, emotional, cognitive, communication, or
developmental disability is eligible to receive services. Diagnoses of those served may
include, but not limited to: brain injury, stroke, spinal cord injury, cerebral palsy,
psychiatric disorder, learning disability, mental retardation, or developmental disability.
We also treat individuals with chronic pain and fibromyalgia.
Admissions decisions are made without regard for an individual's race, color, sex,
religious creed, marital status, national origin, ancestry, mental retardation, past or
present history of mental disability, learning disability, physical disability, age, or sexual
orientation. Services are provided without regard to the income of those served.
Referrals can be made by telephone, fax, mail, or in person. A physician prescription is
required for most services. Our staff will work with a potential patient to secure the
appropriate documentation.
Accessibility
Our facility is on the ground floor and is fully accessible. Parking is free and we are
located on a bus line.
Sage Rehab employs staff fluent in American Sign Language, Spanish, French, and
Arabic.
